Community Wellness Partners announces housing program

CLINTON — Community Wellness Partners has expanded its services to include Permanent Supportive Independent Housing for qualified older adults.

Located on the Presbyterian Homes & Services campus, Supportive Independent Housing at The Meadows allows eligible individuals the opportunity to create a pathway to health and wellness through evidence-based interventions and lifestyle modifications.

In order to qualify for one of the 51 units, an individual must be:

-At risk for homelessness, homeless or inappropriately housed;

-Age 55 or older;

-Medicaid eligible or a current Medicaid recipient;

-Capable of living independently with assistance in order to remain stably housed.

Supportive Independent Living provides residents with care navigators who serve as advocates, helping individuals achieve optimum health and wellness; a wellness coach who assists residents with developing personalized wellness plans; and, reliable transportation to medical appointments and off-site outings.

Amenities in the fully-furnished units include 24-hour emergency maintenance, on-site management and laundry suites on each floor.

Combining more than 150 years of service to the region, Community Wellness Partners is an affiliation of LutheranCare and Presbyterian Homes & Services.

Its faith-based, non-profit organizations offer the most complete continuity of health and wellness services for older adults in Oneida County including Home Care, Supportive Housing, Independent & Assisted Living, Memory Care, Skilled Nursing and more.
